Purposes

The purpose of the treatment of your Personal Data is the provision of health care services, including the integration of your clinical record either physically or digitally. To achieve the aforementioned purposes, the following personal data will be processed: Full name, address, home telephone number, mobile and office telephone number, email, age, date of birth, marital status, as well as full name, address and telephone number of a relative with whom we can communicate in case we cannot locate you and it is necessary to make contact or in case of emergency.

Likewise, for the issuance of the receipt, personal financial data will be processed, such as your fiscal data, the name of the bank and branch where you have a bank account through which you will pay for the services of the Responsible Parties and the last four digits of your bank account number.

Sensitive Personal Data

The following sensitive personal data will be processed by the Controllers: health condition (including the results of medical studies), religious beliefs.

Choices to limit the use or disclosure of your personal information

The Controllers have implemented a mechanism by which you may limit the use or disclosure of your personal data personally to the Controllers, at the Contact Address. To learn about it, please request this information to the Controllers.

Means to revoke consent for the Processing of Personal Data

You may revoke your consent to the processing of your personal data in writing by submitting it to the Contact Address, in accordance with the provisions of the Data Law. The Controllers will respond in writing to your request for renewal, when so requested.

(IV) oppose their processing for specific purposes, as provided in the Data Law.

The Responsible Parties will inform you whether your request is admissible by e-mail or in writing within the following 2 business days, but may request additional information if necessary. In case of being admissible, the Responsible Parties will respond to your request within 15 business days.

Transfer of Personal Data

The Controllers will transfer Personal Data of which you are the Data Subject, only in the event that you request that such Personal Data, including Sensitive Data, be sent to:

  • Insurance company with which you have a policy.
  • Any other Physician or health care professional from whom You have sought services.

Modifications to the Privacy Notice

  • The Responsible Parties reserve the right to make changes or updates to this Privacy Notice at any time and any changes to this Privacy Notice will be announced by means of a notice that will be posted in a visible place at the Contact Address.

  • In the event of a breach of security of your personal data that significantly affects your economic or moral rights, the Responsible Parties will inform you as soon as possible via email or telephone, so that you can take the necessary measures to defend your rights.
